The Role of The Healthy Living Coordination Team

The healthy living coordination team partners with residents to engage in proactive planning to help residents achieve their goal of maintaining their independence and enjoying the highest level of autonomy.

A healthy living coordinator is assigned to each resident at Shell Point and tasked with connecting the resident and their family with a vast array of supportive resources and educational materials as they navigate healthcare decisions or concerns.

It’s a partnership; one that means honest and proactive conversations about each resident’s health and happiness while ensuring that every facet of the health system is used to the resident’s best advantage.

Every situation is unique. In order to provide the best possible services, a healthy living coordinator works to build a two-way relationship and understand what is important to you. This will allow you and your healthy living coordinator to put together the best possible plan for care, identify wellness opportunities and set goals for the future.

Healthy Living Coordination Services

  • Welcome new residents as they arrive to Shell Point, followed by quarterly visits to ensure they are acclimating to the community
  • Visit all independent living residents annually to establish a relationship, build rapport and understand residents’ current and potential care needs
  • Assist residents and their families in developing a plan that promotes autonomy, wellness and healthy living
  • Provide information on resources to support goals
  • Make appropriate referrals and follow up on referrals to evaluate effectiveness
  • Visit with independent living residents and attend care plan meetings
  • Follow up on nurse visits, emergency or hospital discharges, and Larsen Health Center discharges
  • Lead community support groups including the Care Free, COPD, Diabetes, Hearing Enrichment, Heart Healthy, Parkinson’s Enrichment, Connections Circle and Vision Enrichment groups
  • Manage assisted living waiting lists
  • Coordinate transitions to higher levels of care
  • Provide quarterly presentations as part of the Wellness Connection
  • Partner with assisted living directors to offer assisted living seminars and tours
