Jack Adams, MS, CCA-A
Audiologist and owner of Audiology Consultants of Southwest Florida, has been helping people improve their hearing for more than 30 years. At the Arbor Medical Center, he can provide a full range of audiology services, including hearing tests to determine a patient’s level of impairment and the cause of hearing loss. “Hearing tests are simple, quick and painless – and they are covered by Medicare,” said Dr. Adams. “I always encourage people to seek help as soon as they notice any communication problem.”

Dr. Charles Fulton
A fourth-generation native of Fort Myers, has been in private practice for more than 20 years. While working predominantly as a hands-on chiropractor, he will also incorporate other techniques for those who may prefer alternate care options. Dr. John Chouraqui
Native to Paris, France, and now residing in Fort Myers. He earned his Bachelor of Sciences in Biology from Nova Southeastern University, his Masters of Biomedical Sciences from Barry University, and his DMD from Tufts University School of Dental Medicine. He enjoys every aspect of dentistry and the interaction with patients. He especially enjoys getting to know them and learning about them. The best part of his job is the smile on his patients’ faces when he completes a procedure. When he is not helping to keep our patients healthy, he enjoys playing piano, working out, soccer, watching movies, and spending time with his friends and family.
Deborah Streater, DDS
Graduated from the University of North Texas in 1988 and received a dental degree from Baylor College of Dentistry in 1992. Dr. Streater joined Dr. Milsap’s practice in 1996 and began serving the Shell Point community in 2001. Dr. Streater travels to attend classes and obtain updates on implants, as well as cosmetic and family dentistry. She is a member of the FDA, ADA and Lee County Dental Society.

Shannen Vixler, C-APRN
Graduated from Florida Gulf Coast University School of Nursing with both bachelor’s and master’s degrees in nursing, including successful completion of the nurse practitioner program. Her more than 20 years of dermatology clinical experience is reflected in both the quality and thoroughness of her compassionate care. Shannen recommends yearly skin examinations for adults, and has passion for providing quality dermatologic care to the residents of Shell Point.
Allison Tubbs, C-APRN
Graduated at the top of her class from Florida Gulf Coast University with a master’s degree in nursing and has since achieved clinical excellence as a registered nurse in critical and post-anesthesia care. She was also the recipient of the Outstanding Primary Health Care award presented by Sigma Theta Tau International Honor Society. Allison cares for patients by utilizing evidence-based guidelines blended with extensive clinical experience from her more than 20 years of experience in patient care.

Dr. Christopher DiBlasio
Dr. Christopher DiBlasio is a board-certified physician in urology and urologic surgery from Genesis Care in Fort Myers. He received his undergraduate degree from Fairfield University in Connecticut and graduated from St. George’s University School of Medicine in Grenada. He went on to complete an internship in general surgery/trauma and residency in urology at the University of Tennessee College of Medicine & Surgery in Memphis. In addition, he completed a research fellowship in urologic oncology at New York’s renowned Memorial Sloan-Kettering Cancer Center. Born and raised in Long Island, Dr. DiBlasio returned to New York for the first 14 years of his practice. He specializes in minimally invasive surgery for urinary stones, prostate laser surgery, and prostate/kidney cryoablation. He served as Chief of Urology at Northwell Health at Huntington Hospital (NY), Associate Medical Director for the Mount Sinai Health System, and Urology Division Director for Prohealth Care in Suffolk County. In 2021, Dr. DiBlasio was recruited to assume leadership and clinical roles Genesis Care in Fort Myers. Several years ago, Dr. DiBlasio adopted an innovative prostate biopsy approach that employs MRI-fusion technology to minimize infection risk. He pioneered and specializes in MRI-fusion guided cryoablation for prostate cancer, which is an outpatient, cutting-edge evolution in personalized prostate cancer treatment that minimizes potential risks, side effects, and recovery time. In addition, he has extensive experience in female prolapse and incontinence repair, stone surgery, urologic cancer treatment, cosmetics and reconstruction, trauma, and renal transplantation.
Dr. Erin Heitman
Dr. Erin Heitman from GenesisCare Urology is joining Dr. Christopher DiBlasio to serve Shell Point residents from the Specialty Doctors’ Offices in The Arbor. Dr. Heitman is a residency-trained urologic surgeon accredited by the Accreditation Council for Graduate Medical Education and recognized by the American Board of Urology. Originally from Baton Rouge, Louisiana, Dr. Heitman obtained her medical degree from Louisiana State University School of Medicine in New Orleans. She then completed her urology residency at The University of Tennessee Health Science Center in Memphis, Tennessee. Dr. Heitman treats a wide range of general urologic conditions including stone disease, benign prostatic hyperplasia, and urinary incontinence, as well as genitourinary malignancies including kidney, bladder, and prostate cancers.

Dr. Sumeet Shetty
Shell Point has arranged the services of a hospitalist for residents who experience a short-term stay at a Lee Health hospital. A hospitalist is a physician whose primary professional focus is the general care of hospitalized patients. Their responsibilities include patient care, teaching, research, leadership and coordination of care with specialists.
Shell Point residents are encouraged to request Dr. Sumeet Shetty as their preferred hospitalist upon admission to the hospital. With 20 years of experience in patient care and leadership, Dr. Shetty and his team work in the hospital every day, seeing patients, collaborating with nursing staff, following up on diagnostics and addressing medication and treatment issues as they arise.
Once you are discharged from the hospital, Dr. Shetty’s team will collaborate closely with the Shell Point clinical team to see you when you return home. Dr. Bradley O’Neill
Board-Certified Ophthalmologist Dr. Bradley O’Neill has advanced subspecialty training in Cataract/Refractive surgery. Committed to providing the best outcomes to his patients by utilizing the latest technology, Dr. O’Neill provides Laser Cataract surgery as well as LASIK surgery at Eye Centers of Florida. Dr. O’Neill is a member of the American Academy of Ophthalmology and a member of the Florida Medical Society. He received his bachelor’s degree from the University of South Florida and continued to Lake Erie College of Osteopathic Medicine for his doctorate degree, where he graduated with honors. He completed his internship and residency at the Philadelphia College of Osteopathic Medicine, where he served as the Chief Resident and received much of his instruction from the world-renowned faculty of Wills Eye Hospital. Dr. David Eichten
Dr. David Eichten attended medical school at Kansas City University of Medicine and Biosciences. During that time, he spent two years on clinical rotations in Fort Myers, where he worked with Dr. Humbert and Dr. Fenning at Joint Implant Surgeons of Florida. He also spent time at Shell Point working with resident physicians and completed an extra year of fellowship training in hip and knee joint replacement. Dr. Eichten takes pride in providing the best in joint replacement care and helping patients get rid of pain so they can return to the activities they love.

Dr. Nelson Hulbert is a Podiatry Specialist in Fort Myers, Florida. He graduated with honors from Kent State University College Of Podiatric Medicine in 2012. Having more than 11 years of diverse experiences, especially in podiatry. Dr. Hulbert works with many other doctors and specialists in the area. At Shell Point, he joins Dr. Curtis Skupny, who graduated from Kent State University College of Podiatric Medicine in 1985 and has been a part of Shell Point’s specialty clinic for many years. Dr. Robert Dushack
Dr. Robert Dushack is a U.S. Navy veteran of the Gulf War. He graduated from Saint Vincent College in Latrobe, Pennsylvania, where he received a bachelor’s degree in biology. Dr. Dushack completed his formal medical training at Temple University School of Podiatric Medicine, and also completed an intensive residency program at the University of Pittsburgh, Mercy Hospital. Dr. Dushack is trained in the medical and surgical care of all lower extremity ailments. Dr. Dushack lives in Fort Myers, where he has his own practice, APEX Foot & Ankle Center, LLC.
