Support and Care for Memory Loss

The focus of Shell Point’s Cognitive and Behavioral Health Program is to serve God and care for His people throughout the continuum of care through comprehensive, preventative and proactive approaches that engage residents and families in an active role in their health and well-being. The foundation of the program is an integrated care team, resident and family engagement, a comprehensive care plan and a robust variety of education, support and resources. The program also seeks to foster community partnerships and engage innovative approaches to support excellence in care and further position Shell Point as a leader in senior care. 

Connections Memory Care at Shell Point

The Connections program is designed to support and care for individuals experiencing memory loss, Alzheimer’s disease and other dementias throughout the continuum of care. Cognitive health is a key determinant of satisfaction, engagement and independence throughout life. Connections offers high quality programs focused on maintaining cognitive health and early detection of brain diseases. We offer extensive resources, services and the collaboration of an interdisciplinary team of healthcare professionals to meet a wide array of memory care needs.

Healthy Living Coordination at Shell Point is the best place to start. The Healthy Living Coordinators have the skills and experience to help you navigate the resources within Shell Point and beyond. In addition, they collaborate with an interdisciplinary team of healthcare professionals to provide comprehensive assessment of an individual’s needs. The interdisciplinary team includes our Behavioral Health and Primary Care providers, Home Care, Assisted Living and Skilled Nursing teams.
