The healthy living coordination team partners with residents to engage in proactive planning to help residents achieve their goal of maintaining their independence and enjoying the highest level of autonomy. A healthy living coordinator is assigned to each resident at Shell Point and tasked with connecting the resident and their family with a vast array of supportive resources and educational materials as they navigate healthcare decisions or concerns.
